Information we collect

The contact form collects the name, contact information, optional service preference, consent, and optional message you choose to provide. Please do not submit medical or clinical details through this website.

How it is used

Submissions are used only to respond to your inquiry. They are emailed to the practice and stored in a secure, timestamped log as a delivery failsafe. Information is not sold.

To limit automated spam, the log also records the time of submission and a one-way hash of the network address it came from. The address itself is not stored.

Website analytics

This site uses Google Analytics 4 to measure website visits, successful contact submissions, and clicks on phone links. Form contents and service selections are not included in the analytics events we send. Analytics may be linked to Google Ads to measure advertising performance. Advertising personalization and Google signals are disabled in our website tag.

Clinical privacy
